- Nautilus — Nautilus is a hook and engage support who locks down a single target with Dredge Line hook, Staggering Blow root, and Depth Charge knockup. Milio counters Nautilus because Breath of Life cleanses the root from Staggering Blow and the stun from Dredge Line, while Warm Hugs shields absorb the damage from Nautilus's combo. Nautilus relies on layering CC to keep a target locked down long enough for his ADC to kill them, but Milio's R removes all of that CC instantly. The 65% tenacity afterward means even Depth Charge's knockup follow-up is shortened significantly.

- Lulu — Lulu is an enchanter support who polymorphs enemy threats and empowers carries with attack speed steroids and shields. The matchup is even because both enchanters excel at protecting their carries — Lulu's polymorph shuts down divers while Milio's Breath of Life cleanses CC. Lulu's Whimsy attack speed buff competes with Milio's Cozy Campfire range buff as carry steroids, and both scale similarly with enchanter items. The lane outcome depends on which ADC can capitalize better on their support's buffs and which jungler ganks more effectively.
- Nami — Nami is an enchanter support who combines healing, poke, and crowd control with Tidecaller's Blessing and Tidal Wave. The matchup is even because Nami's sustain from W bounce healing matches Milio's Cozy Campfire healing, and both champions empower their carries with steroids — Nami's E grants on-hit slow and damage while Milio's W grants attack range. Nami has more lane poke pressure with W bounces and E-empowered autos, but Milio's Breath of Life can cleanse Nami's Tidal Wave engage in teamfights, keeping the late-game power balanced.
- Thresh — Thresh is a hook support who combines pick potential with lantern utility and Flay peel. The matchup is even because Milio's Breath of Life can cleanse Thresh's hook and Flay stun, but Thresh's lantern brings allies that Milio's single knockback cannot stop alone. Thresh's hook threat forces Milio to position carefully in lane, but Milio's Warm Hugs shields and Cozy Campfire healing sustain through Thresh's poke. In teamfights, Milio's R counters Thresh's CC chain but Thresh's utility with lantern provides value that Milio cannot replicate.

- Vel'Koz — Vel'Koz is an artillery mage support who deals heavy sustained damage from extreme range with Plasma Fission, Void Rift, and Lifeform Disintegration Ray. Vel'Koz counters Milio because his long-range poke deals damage that Milio's shields and healing cannot keep up with — Vel'Koz chunks the ADC from ranges where Milio's Cozy Campfire healing cannot outheal the damage, and his true damage passive from Organic Deconstruction ignores any resistances Milio provides. Milio's Breath of Life does not help against Vel'Koz because Vel'Koz's damage does not rely on crowd control to kill — he simply out-damages Milio's protection.
- Karma — Karma is a poke-enchanter hybrid who deals significant early damage with Mantra-empowered Inner Flame while providing shields and speed to her own team. Karma counters Milio because her poke damage in lane is far stronger than Milio's Q, her Inspire shield matches Milio's Warm Hugs, and her Mantra-empowered abilities give her burst damage and utility that Milio cannot match. Karma wins lane trades handily because her damage output overwhelms Milio's healing and shielding, forcing Milio's ADC to fall behind in farm and pressure.
- Sona — Sona is a scaling enchanter who provides team-wide auras for damage, movement speed, and damage reduction. Sona outscales Milio because her passive auras buff all nearby allies simultaneously, and her Crescendo ultimate provides a team-wide stun that Milio's team cannot avoid. While Milio can cleanse Crescendo with Breath of Life, Sona's overall scaling with enchanter items provides more total value through constant team-wide auras, healing, and shields that exceed what Milio can output in extended late-game teamfights.
